Animal Emergency Clinic of the Fraser Valley welcomes the transfer of patients to our Emergency and Internal Medicine services, as well as those patients requiring 24-hour care.

Emergency Transfers

To transfer an emergency patient, please call us at 604-514-1711 so that we can prepare for your patient’s arrival.

Internal Medicine Transfers

To refer a patient to our Internal Medicine service, please fill out the form below. You can also download the referral form and send it with the records to [email protected].

For an efficient transfer, we recommend preparing the following:

• Any lab work – CBC, chemistry panel, urinalysis, fecal, other – including previous blood work for comparison

• Any biopsy, cytology reports

• Any surgery reports

• All previous diagnostic imaging interpretation reports, including radiographs, ultrasound, CT, and MRI

• All referral reports if the patient has been seen by a specialist
